TOM LASTUVKA, QKA Co-Founder
Tom co-founded Qualified Plans Consultants, LLC (QPC) in 2003 and has over 17 years of experience working with retirement plans. He is responsible for the firms’ day-to-day operations, including employee management, administration, compliance, consulting, and with marketing the firm's services in eastern Iowa, Illinois, and Minnesota.
Tom has earned his Qualified 401(k) Administrator (QKA) professional designation sponsored by the American Society of Pension Professionals and Actuaries (ASPPA), and is currently working towards his Certified Plan Financial Consultant (CPFC) designation. He holds a Bachelor of Business Administration degree from Iowa State University, Ames, Iowa, with a major in Finance.
Prior to starting QPC, Tom held a similar position with a local third party administration firm. Before that, he worked as a financial planner and Registered Representative with Waddell & Reed in Des Moines, Iowa. He also served as an Administration Specialist, Training Specialist, Assistant Manager, and Manager at The Principal Financial Group in Des Moines, Iowa, in Defined Contribution administration divisions that handled 401(k), 403(b), 457 and nonqualified retirement plans.
MICHAEL (MICK) FOUTS Co-Founder
Mick co-founded Qualified Plans Consultants, LLC (QPC) in 2003 and has over 15 years of experience working with retirement plans. He is primarily responsible for the firms’ marketing and sales efforts which entails calling on a variety of networks and developing relationships with investment advisors, investment companies, wire houses, banks, and independent broker-dealers.
Mick holds a Bachelor of Arts degree from Westmar College, LeMars, Iowa, with a double major in Finance and Economics and a minor in Business Management.
Prior to starting QPC, Mick held a similar position with a local third party administration firm. Prior to that, he worked as a Pension Marketing Consultant at WellMark Capital Value, a former division of Blue Cross/Blue Shield of Iowa, in Des Moines, Iowa. He also served as an Administration Specialist at The Principal Financial Group in Des Moines, Iowa, in a division that primarily handled 401(k) and other qualified retirement plans.
MITZI K. NELSON, QKA Senior Retirement Plan Consultant
Mitzi joined Qualified Plan Consultants, LLC in August, 2004, after serving as a Senior Conversion Manager at ADP TotalSource in Miami, Florida. Her 20+ years of experience includes work in the following areas:
• Plan mergers, terminations and conversions • Plan design consultation and legal compliance • Discrimination testing for Defined Contribution Plans • Preparation of annual reporting and disclosure • Implementation of new employee benefit plans
While at ADP TotalSource, Mitzi played a key role in consulting assignments for professional firms and other plans sponsors converting to the ADP TotalSource 401(k) Multiple Employer Plan. In addition, she helped clients in correcting plan violations, and provided technical support for in-house and outside sales teams.
Prior to joining ADP TotalSource, Mitzi was a Manager of the pension consulting team with MetLife, Inc. Her role emphasized the conversion of new clients coming into MetLife, Inc. and the underwriting process to determine client service level.
At QPC, Mitzi's role is working with our largest and most complex clients with ongoing plan administration.

DONALD LOVE Senior Retirement Plan Consultant
Don joined Qualified Plan Consultants, LLC (QPC) in September 2006 as a Senior Retirement Plan Consultant. His responsibilities at QPC include the ongoing administration, compliance, and documentation services for our larger and more complex clients.
Don has over 19 years of professional experience working with qualified retirement plans. Don is currently working towards earning his QKA (Qualified 401(k) Administrator) professional designation sponsored by the American Society of Pension Professionals and Actuaries. Don holds a Bachelors of Science in Business Administration degree with a major in Finance and a minor in Accounting from the University of Nebraska at Lincoln.
Prior to joining QPC, Don held similar positions with regional and national third party administration firms in Omaha, Nebraska. He also served as an Employee Benefits Administrator, Trust Officer and Senior Trust Officer for First National Bank of Omaha’s Trust Department. Don has a comprehensive background in qualified retirement plans, encompassing consulting, administration, plan design, management, training and compliance resolution.
ABBIE BUNGE Retirement Plan Consultant
Abbie joined Qualified Plan Consultants in October 2006 as a Retirement Plan Consultant after graduating from Iowa State University in Ames, Iowa, with a Bachelor of Science degree, majoring in Finance and International Business.
Abbie's responsibilities include day-to-day plan administration, compliance, and plan document services for a diverse group of retirement plan clients.
She is currently working towards her QKA (Qualified 401(k) Administrator) professional designation sponsored by ASPPA (American Society of Pension Professionals and Actuaries).
CARLA RUBLE Retirement Plan Consultant
Carla joined Qualified Plan Consultants in October 2006 as a Retirement Plan Consultant. Her responsibilities include day-to-day plan administration, compliance, and document services for a diverse group of clients.
She is working towards earning her QKA (Qualified 401(k) Administrator) professional designation sponsored by ASPPA (American Society of Pension Professionals and Actuaries).
Carla holds a Bachelor of Science degree in Accounting with a minor in Business Management from Upper Iowa University.
Prior to joining QPC, Carla worked for another regional third party administration firm for 9 years.
TORI PANEK Retirement Plan Consultant
Tori joined Qualified Plan Consultants in October 2006 as a Retirement Plan Consultant. Her responsibilities include day-to-day plan administration, compliance, and plan document services for a diverse group of clients.
She is working towards her QKA (Qualified 401(k) Administrator) professional designation sponsored by ASPPA (American Society of Pension Professionals and Actuaries). Tori holds an Associate Degree in Business Management from DMACC.
Prior to joining QPC, Tori worked for another regional third party administration firm for 4 years and at Principal Financial Group for 5 years working with retirement plans.
DARLA HUNZELMAN Retirement Plan Consultant
Darla joined QPC in January, 2007 as a Retirement Plan Consultant. She has over 14 years of experience working with retirement plans. Her responsibilities include day to day administration for an assigned block of clients, plan design assistance, compliance, and plan document services.
Prior to joining QPC, Darla held a similar position with a local third party administration firm. Before that, she worked for The Principal Financial Group, Des Moines, Iowa in the Contract Compliance and Defined Contribution Administrative divisions for 12 years.
She is working towards her QKA (Qualified 401(k) Administrator) professional designation sponsored by ASPPA (American Society of Pension Professionals and Actuaries). Darla holds an Associate of Arts Degree, with an emphasis on Psychology, from Marshalltown Community College.
JAMES WALKER
Regional Vice President - Sales
James joined Qualified Plan Consultants, LLC (QPC) in February of 2007 as a Sales & Marketing Consultant. He has over 9 years of retirement plan experience, and is responsible to develop relationships with investment advisors, service providers, wire houses, banks, and independent broker-dealers in Illinois, Wisconsin, and Minnesota.
James holds a Bachelor of Business Administration from Western Michigan University, Kalamazoo, Michigan, with a major in Finance and minors in Accounting and General Business.
Prior to joining QPC, James was a retirement plan wholesaler for Great-West Retirement Services in the territory of MN & IA. He also served as an inside wholesaler with John Hancock Retirement Services in Chicago, IL where he marketed their retirement plan products.
CORBON KINNEY New Business Manager
Corbon joined Qualified Plan Consultants in April 2007 as the New Business Manager. He is responsible for the internal oversight of all new retirement plan business to the firm, including plan design, plan transition, document drafting services, and sales and marketing support.
Corbon graduated from Iowa State University in 1996 with a Bachelor of Science in Education and spent 4 years as a middle school teacher and coach. Prior to joining QPC, he worked with qualified retirement plans at the Principal Financial Group and DeWaay Capital Management. He has a strong background in client transition management, underwriting, and marketing.
SUE ANN LASTUVKA Bookkeeper
Sue Ann has performed bookkeeping services for Qualified Plan Consultants, LLC (QPC) since the firm began in September 2003. She is responsible for all accounts receivable, accounts payable, state and federal tax forms, tax deposits and payroll.
Sue Ann holds a Bachelor of Business Administration degree with a major in Psychology from Briar Cliff University, Sioux City, Iowa.
Prior to working for QPC, Sue Ann has over 16 years of experience working with retirement plans in various Pension divisions at the Principal Financial Group in Des Moines, Iowa, most recently as Manager and Senior Business Coordinator.
CHRIS STONE Retirement Plan Administrator
Chris joined QPC in June 2007 as a Project Assistant / Account Coordinator II, and was promoted to Retirement Plan Administrator on September 1, 2008.
His responsibilities at QPC include day to day administration for an assigned block of clients, plan design assistance, compliance, and plan document services, discrimination testing and Form 5500 preparation. Additional duties include processing plan distributions and loans, working directly with many different clients regarding our services, and assisting staff with a multitude of projects.
Prior to joining QPC, Chris had experience in Operations Management and Sales.
CHELSEY STANLEY Administrative Assistant / Receptionist
Chelsey joined QPC in June 2007 as an adminstrative Assistant and Receptionist, located in our Des Moines, IA office.
Her responsibilities include answering incoming phone calls, greeting guests, handling incoming and outgoing faxes, filing, entering new client information into our databases, and preparing marketing materials and new client binders. She also assists staff with numerous projects as needed.
Prior to joining QPC, Chelsey worked in Financial Services, Sales, and Customer Service.
TAMMY WILLIAMS
Senior Retirement Plan Consultant
Tammy joined Qualified Plan Consultants, LLC (QPC) in December, 2007 as a Senior Retirement Plan Consultant. She has over 10 years of experience working with retirement plans including both Defined Contribution and Defined Benefit Plans. Her responsibilities include compliance, document services, and administration for our clients.
Tammy is a designated member of the American Society of Pension Professionals and Actuaries (ASPPA) and has earned professional designations as a Qualified Pension Administrator (QPA) and Qualified 401(k) Administrator (QKA). She is currently working toward her Certified Pension Consultant (CPC) designation. Tammy graduated from Dana College with a degree in Secondary Math Education.
Prior to joining QPC, Tammy held similar positions with regional and national third party administration firms in Omaha and Lincoln, Nebraska. Tammy has a comprehensive background in qualified retirement plan consulting, plan design, compliance, annual reporting, and administration. Prior to her career in the qualified retirement plan industry, she taught middle school mathematics for Omaha Public Schools for 3 years.
DEYANN WHITTIE Document Specialist / Account Coordinator II
Deyann joined Qualified Plan Consultants, LLC (QPC) in September, 2008, as a Document Specialist/Account Coordinator II. Her responsibilities at QPC include processing plan distributions and loans, in addition to assisting with plan document reviews and restatements.
Deyann has over 20 years of professional experience working with qualified retirement plans. She graduated from the University of Northern Iowa with a Bachelor of Arts degree in Accounting and has her CPA certification.
Prior to joining QPC, Deyann was a Trust Officer and Assistant Vice President for U.S. Bank’s Institutional Trust and Custody division. Deyann’s extensive background in qualified retirement plans includes plan administration, document preparation and participant recordkeeping.
ANDREW FOLTZ Account Coordinator II
Andrew joined Qualified Plan Consultants in September 2008 as a Retirement Account Coordinator. He graduated from Luther College in Decorah, Iowa, with a Bachelor of Arts degree, majoring in Mathematics and Physics.
Andrew’s responsibilities include vesting calculations, distribution processing, assistance with plan administration, preparation of annual compliance forms, and client correspondence.
Prior to joining QPC, Andrew worked as an adjunct math instructor for Des Moines Area Community College, Boone Campus.
KYLE FAABORG Account Coordinator II
Kyle joined Qualified Plan Consultants, LLC (QPC) in September 2008 as an Account Coordinator. Kyle recently graduated from Wartburg College in Waverly, Iowa with Bachelor of Arts degree, majoring in Business Finance and Business Management.
Kyle’s responsibilities include assisting with the daily administration of qualified plans, including distributions, compliance testing, and government reporting. Additional duties include assisting Retirement Plan Consultants, calculating contribution and allocations, and monitoring loans. Prior to joining QPC, Kyle had experience in Banking and Accounting.
